The CSV should not have the -R.  The CSV header needs to be be FIRSTNAME.  The -R goes into the PSD, not the CSV.

It is set up this way so that you can use the same CSV for different templates that have different resizing options.  Say for example, you are running a batch that uses the TEMPALTE FILE column, and you specify multiple PSDs in the column that have different resize options set up in the PSDs. There is no way it could work if the resize was also specified in the header.
