I found 2, maybe 3 issues in your CSV.
1. The Team filename column head is incorrect. It needs to be TEAMFILE, not TEAM.
2. The Name O’Rourke has an invalid character for ‘. This may be messing up the data parsing. Make sure to set the character set to UTF-8 when saving the data file. It probably defaulting to Western Europe or something like that. If saving the file from excel, set the export options to UTF-8 when saving the csv. When not using UTF-8, special characters can mess up the data. You can also try removing that character.
3. The filenames listed have $ in the name. Does the filename actual have $ in the name? Just wondering if there was a data entry error creating the $ in the CSV file.
