The CSV headers are not Space or Case sensitive so it should work with or without a space. Only the PSD template layers are case/space sensitive.
For the subfolder, you can’t make a path with / or \. It needs to be just sub folder name only in the cells. So if you are using / or \ then remove that.
I will test tomorrow to see if something changed with the Photoshop security not allowing it to create subfolders.
