First try restarting the computer. If it still happens then try resetting the Photoshop preferences and then restart Photoshop. Sometimes the prefs files get corrupted and cause strange issues.
if it still happens after resetting the preferences then I will need you to send me the PSD, CSV, and jpg image. Too difficult to troubleshoot without being able to test myself.
