Also, you need to make sure the text layers are all set up as point text, not paragraph text. After you have the text layers named properly, click on the convert to point text button under the PSD setup in SPA. That will do it automatically for all of the SPA text layers.

last thing is make sure you have all of the fonts installed. The layer in your screen shot had the ! warning on it which typically mean s you are missing a font.

If these are not the issue then you can send the PSD and CSV to damon@pixnub.com then I can look at it.  You can downsize the PSD to something really small, like 750 pix to make it small enough to send in an email.
