I got your files and message. Glad you got the CSV to finally load. What issue did you find casing it not to load?

Regarding it not replacing the text, the layer names in your PSD are incorrect. You have FIRSTNAME2 and LASTNAME2 as the layer names. The names must be FIRSTNAME and LASTNAME.

WITH SPA 8 and 9, there is a standard list of names that must be used. Without getting into all of the technical detail of why, having a set list of names allows it to run much faster than if the user makes up their own names. This is the list of valid names for the text layers.
