Then why do you have 3 background template PSD files listed?
You need one PSD template, not 3 different templates. You add in a setup layer set for Player 1, 2 and 3 on the same PSD. So the template will have 3 player setups. You just position scale, and move each player setup where you want it.
So in your case, you have only 1 row per composite. 1 row = 1 final composite
See this webpage. Scroll about 1/2 way down and there is a screenshot showing Player File 1/2/3 columns in the CSV. that is how you need to set up your CSV.
Also, I recommend setting up your CSV headers using the built in CSV starter in 9.2. This will make sure you have the headers correct.
