I know you already mentioned those things. I just wanted to double check to make sure I understood.

So at this point, I think you need to contact Adobe and have them fix Face Aware Liquify. Whatever they do to fix that will fix SPA.

please let me know what they have to do. I would like to know for future reference. Because I really have no idea what to check. All I know for certain is the issue is buried somewhere in Photoshop.
